In consideration of the minor participant being permitted to participate in PTP Elite Football Prep Academy training, instruction, 7-on-7 preparation, football IQ sessions, speed and agility work, conditioning, evaluations, meetings, events, and related activities, I, the undersigned parent or legal guardian, agree to this Registration and Liability Waiver on behalf of myself and the minor participant.

I understand that football training and athletic development activities involve inherent and other risks, whether known or unknown, including but not limited to running, jumping, cutting, backpedaling, throwing, catching, defending, accidental contact, collisions, falls, equipment contact, facility conditions, outdoor heat, dehydration, illness, communicable disease exposure, sprains, strains, fractures, dental injuries, eye injuries, head injuries, concussion, heat illness, cardiac events, paralysis, permanent disability, and death.

I voluntarily assume all risks connected with participation, including risks arising from the ordinary negligence of PTP Elite, its owners, coaches, trainers, staff, contractors, volunteers, sponsors, partners, facility owners, facility operators, and their respective representatives, to the fullest extent permitted by Arizona law. This release is not intended to release claims that cannot legally be released, including gross negligence, willful, wanton, malicious, reckless, or intentional misconduct where such release is not permitted by law.

To the fullest extent permitted by law, I release, waive, discharge, hold harmless, defend, and indemnify PTP Elite and the released parties from claims, demands, causes of action, damages, costs, and expenses arising out of or related to the participant's participation, presence at any activity location, use of equipment, instruction, supervision, emergency response, or travel to and from activities, except to the extent prohibited by law.

I authorize PTP Elite staff to obtain emergency medical care for the participant if I cannot be reached. I understand that I am responsible for medical costs, insurance deductibles, and related expenses. I certify that the participant is physically able to participate and that I have disclosed all relevant medical conditions, medications, allergies, restrictions, and prior injuries.

I understand PTP Elite does not guarantee team placement, school placement, playing time, recruiting results, scholarship offers, academic eligibility, or athletic outcomes. I agree that the participant must follow coach instructions, safety rules, facility rules, hydration guidance, and conduct expectations, and that PTP Elite may remove a participant for unsafe or disruptive conduct.

I consent to electronic signature and electronic storage of this waiver. I understand that submission may record the waiver version, timestamp, browser/user-agent information, and technical submission data for audit and recordkeeping purposes. This agreement is governed by Arizona law to the fullest extent permitted, and any unenforceable provision shall be severed without affecting the remaining provisions.

Required Acknowledgements I have read and understand the risks of football training and athletic activity, including serious injury or death. I agree to the assumption of risk, release of liability, hold harmless, defense, and indemnity language above. I authorize emergency medical care and certify the medical information provided is complete and accurate. I agree that the athlete will follow coach instructions, safety rules, and facility rules. Optional: I grant PTP Elite permission to use photos or video of the athlete for program, social media, and promotional purposes.
